Presentamos 3 casos de edema posneumonectomía en los que se procedió a evaluar la situación hemodinámica mediante catéter de Swan-Ganz. La presión capilar pulmonar efectiva fue medida con un sencillo método a partir de la curva de oclusión de la arteria pulmonar. En los 3 casos estudiados se objetivó una presión capilar elevada con valores de presión de enclavamiento normales. Este hecho sugiere que el incremento en la presión neta de filtración como consecuencia de una situación de hiperaflujo es el mecanismo patogénico desencadenante del edema posneumonectomía. Destacamos que la medida de la presión capilar pulmonar efectiva permite una aproximación más exacta que la aportada por presión de enclavamiento a la situación hemodinámica real de los pacientes con edema posneumonectomía.

Three cases of postpneumonectomy edema, in which hemodynamic stability was monitored with a Swan-Ganz catheter, are described. Measurement of pressure in the pulmonary capillary was based on the pulmonary artery occlusion curve. High capillary pressure and normal wedge pressure were observed in all 3 cases, suggesting that the rise in net filtration pressure as a consequence of excess flow is the pathogenic mechanism that triggers edema after pneumonectomy. We emphasize that measurement of effective pulmonary capillary pressure allows for more accurate assessment of the hemodynamic status of such patients than does the measurement of wedge pressure.
