Con el fin de valorar la afectación de la función pulmonar (FP) en la espondilitis anquilosante (EA), se estudiaron 93 pacientes diagnosticados de EA, que se clasificaron en 4 grupos de menor a mayor severidad. El estudio de la FP incluyó en todos ellos una espirometría con test broncodilatador y, en parte de la serie volúmenes pulmonares, capacidad de difusión (DLCO) y gases arteriales en reposo respirando aire y con FiO2 1,0 D(A-a)O2. Del total, 44 (47%) presentaron alguna alteración de la FP: restrictiva 22 (24%), obstructiva 17 (18%) y mixta cinco (5%). Los valores de PaO2 en reposo estuvieron en general en el rango de referencia, y en ningún caso se observó hipercapnia. La D(A-a)O2 fue > 90 mmHg en el 40% de los casos. La TLCO fue normal en los 40 pacientes en que se midió. Observamos diferencias estadísticamente significativas al comparar los pacientes con EA según su severidad, con un mayor deterioro funcional respiratorio en los grupos de EA más evolucionada. También destacamos la relación entre EA y tabaco como factor coadyuvante a la intensidad de la alteración de FP observada.

In an attempt to evaluate the pulmonary function in ankylosing spondylitis, 93 patients with spondylitis were included in a study and divided in four groups, according to the severity of the spondylitis. The pulmonary function study included spirometry with bronchodilatation test and in part of the pulmonar volume series, diffusion capacity (DLCO), arterial gases while at rest and breathing air and with FiO2 1.0 D(A-a)O2. Out of the total, 44 (47%) patients presented alterations in FP: restriction in 22 (24%), obstruction in 17 (18%) and a mixture of both in 5 (5%). PaO2 values at rest generally were within normal limts and no hypercapnia was seen any of the patients. In 40% of all cases D(A-a)O2 was > 90mmHg. TLC was normal in all 40 patients in whom it was measured. There were statistically significant differences between the spondylitis groups with those with more advanced disease showing more deterioration in respiratory function. The relationship between spondylitis and tobacco smoking as an factor in the severity of observed alterations in pulmonary function is underlined.
