La estenosis subglótica en el niño puede ser congé-nita o adquirida, siendo en la mayoría de los casos secundaria a la intubación endotraqueal prolongada. La estenosis subglótica puede manejarse médicamente, mediante resección endoscópica por electro-coagulación, crioterapia, láser o por varios procedimientos quirúrgicos.

En los tres últimos años hemos tratado nueve pacientes con estenosis subglótica entre los ocho días de vida y los catorce meses de edad, siendo en un sólo caso su etiología congénita. Los restantes se debieron a intubación prolongada. Siete de los nueve pacientes fueron tratados con éxito; tres médicamente, tres mediante electrocoagulación y uno con resección quirúrgica endoscópica de membrana subglótica. Los dos restantes precisaron traqueotomía, uno de ellos tras electrocoagulación y otro por reestenosis de la anastomosis tras resección quirúrgica del área estenótica congénita. La estenosis subglótica del niño es un problema complejo y para seleccionar el tratamiento es preciso comprender la evolución de la enfermedad, conocer su estadio histopatológico y las diferentes modalidades terapéuticas.

Subglottic stenosis in the child may be congenital or ad-quired. Most of the times this stenosis is secondary to pro-longed tracheal intubation.

Subglottic stenosis may be managed medically, as well as by endoscopic resection using electrocoagulation, criotherapy or laser ray, or by several surgical procedu-res.

In the last three years we have treated nine patients with Subglottic stenosis aged between 8 days and 14 months. Only in one occasion was the disease of congenital etiology. The remaining were due to prolonged intubation.

Seven out of the nine patients were treated success-fully; three of them by medical means, three by electrocoagulation and one by surgical endoscopic resection of a Subglottic membrane. Two patients required tracheo-tomy, one after electrocoagulation and one because of a restenosis of the anatomosis after surgical resection of the congenital stenotic region.

Subglottic stenosis is a complex condition. The choice of its treatment demands understanding of the outcome of the disease as well as awareness of the histopathologi-cal stage of the disease and the different therapeutic ap-proaches.
