Toxicidad pulmonar aguda por carbamazepina: A propósito de un caso

La presencia de toxicidad pulmonar por carbamazepina parece tratarse de un proceso no dependiente de la dosis, que cursa con infiltrados pulmonares, eosinofilia y exantema cutáneo. Comunicamos un caso de presentación aguda, con evolución favorable tras supresión del fármaco y tratamiento con corticoides. La práctica de un lavado broncoalveolar demostró una inversión del cociente linfocitario CD4/CD8, lo cual, sumado a otras experiencias comunicadas en la literatura, parece apoyar el diagnóstico de toxicidad pulmonar por fármacos.

Pulmonary toxicity caused by carbamazepine seems to be a non dose-dependent process involving pulmonary infiltrates, eosinofilia and skin rash. We report a case with acute presentation that resolved successfully after the drug was withdrawn and the patient was treated with corticosteroids. Bronchoalveolar lavage showed an inversion of the lymphocyte ratio CD4/CD8 supporting the suggestion by other authors that drug-induced pulmonary toxicity.
