Se presenta un caso clínico de síndrome mediastínico, por tumor germinal con manifestaciones importantes de insuficiencia respiratoria y determinaciones elevadas de alfa-fetoproteína y gonadotrofina coriónica.

El estudio anatomopatológico demuestra la existencia de un tumor germinal con diferentes tipos de células malignas, diferenciando las estructuras histológicas a un diagnóstico de teratoma.

Se comenta la evolución clínica, el tratamiento radioterápico y por quimioterapia realizado.

A case of mediastinal syndrome caused by a germinal tumor with severe respiratory insufficiency and high alpha-fetoprotein and chorionic gonadotrophin levels is reported. Pathology studies revealed a germinal tumor composed of different types of malignant cells; differential histology studies established a teratoma.

The clinical course and radiotherapy and chemotherapy are described.
