A pesar de ser una enfermedad conocida por nosotros desde hace 100 años y a pesar de los continuos avances bioquímicos e inmunológicos de la pasada década, en la práctica médica diaria continuamos enfrentándonos a la sarcoidosis con perplejidad. Probablemente, la primera dificultad para su comprensión provenga de la falta actual de unas bases diagnósticas precisas. Para ilustrar este problema, exponemos 2 casos de granulomatosis pulmonar de tipo sarcoideo muy distintos entre sí y de características excepcionales.

Tras comentar sus aspectos relevantes, la metodología y problemática diagnóstica, concluimos que en este tipo de granulomatosis el diagnóstico de sarcoidosis debe de ser planteado únicamente con carácter provisional.

Is spite of being a disease known for up to 100 years, and the continuous biochemical and inmunological progress over the last decade, sarcoidosis is still a perplexing problem in daily clinical practice. Probably the main difficulty for the understanding of this disease results from the lack of accurate diagnostic tools for the time being. In order to illustrate this problem we report two cases of sarcoid-like pulmonary granulomatosis very different from each other and associated with unusual findings. Following a discussion on the most relevant aspects, methodology and diagnostic problems, we conclude that in this type of granulomatosis the diagnosis of sarcoidosis should only be established on a preliminary basis.
