Se estudió la alteración pulmonar en 28 pacientes consecutivos con lupus eritematoso sistémico (LES), no seleccionados por sus síntomas respiratorios. Las pruebas de función pulmonar muestran alguna alteración en 25 pacientes (75%) sin relación con la historia clínica o radiografía torácica. La capacidad de difusión estuvo reducida en 16 casos (57%), y una disminución de la capacidad pulmonar total (TLC) estuvo presente en 2 pacientes. La medida de las presiones máximas estáticas revelan debilidad muscular inspiratoria y espiratoria en 5 casos (18%). Esta alteración puede ser considerada probablemente como parte de una afectación muscular subclínica debida al LES, que sin embargo, afectaría predominantemente a los músculos respiratorios. Esta debilidad no estuvo relacionada con el uso de corticoides o inanición generalizada. La reducción de la TLC presente en el LES tendría como patogenia probable la debilidad muscular respiratoria.

The degree of pulmonary impairment was studied in 28 consecutive patients with systemic lupus erythematosus (LES) not selected by respiratory symptoms. Pulmonary function tests revealed abnormalities In 25 patients (75%). Those findings showed no relationship with the clinical history or the thorax x-ray examination. Diffusion capacity was reduced in 16 cases (57%), and two patients showed a reduced total lung capacity (TLC).

Be of static maximum pressures revealed inspiratory and espiratory muscular weakness in 5 cases (18%). Such abnormalities may be considered probably as a part of a subclinical muscular involvement due to LES, with a predominancy for respiratory musdes. The muscular weakness was not related to corticosteroid treatment or generalized inanition. The most probable pathogenesis of the decreased TLC seen in patients with LES would be a respiratory muscles weakness
