Embolizacion de las arterias bronquiales en la hemoptisis mayor y masiva

Siete pacientes afectos de hemoptisis mayor o masiva fueron tratados mediante embolización de las arterias bronquiales. Tres pacientes presentaban un estudio de función pulmonar que contraindicaba la resección pulmonar quirúrgica, otros tres pacientes carecían de una localización exacta del origen de la hemoptisis y un último paciente padecía el debut de una tuberculosis pulmonar. En todos los enfermos se logró el control inmediato del sangrado, aunque en dos pacientes la hemoptisis recurrió de forma severa a las 24 horas y a los 5 días de la embolización respectivamente, por lo que fueron sometidos a cirugía de resección pulmonar. En los restantes pacientes no ha recurrido la hemoptisis en un período de 2 a 21 meses (media 10 meses). Se concluye que la embolización de las arterias bronquiales es un tratamiento eficaz para el control inmediato de la hemoptisis mayor y/o masiva y se discute su papel en el controvertido manejo de esta patología.

Seven patients suffering from massive and gross hemoptysis underwent embolization of bronchial arteries. Three patients presented pulmonary function tests which results contraindicated surgical resection of the lung, in other three patients, the localization of the bleeding point could not be achieved, and the remaining patient presented hemoptysis as the first manifestation of a tuberculosis. Bleeding immediately ceased in all the patients, but in two of them, the hemorrhage recurred severely 24 hours and 5 days after the embolization, respectively, for which reason, they underwent pulmonary resection. In the remaining patients, hemoptysis did not recur in a period oscillating from 2 to 21 months (mean, 10 months). It is concluded that embolization of bronchial arteries is an efficacious treatment for achieving immediate stopping of massive and/or gross hemoptysis and the role of this treatment in the management of such disorder is discussed.
