Resumen

La exploración de la regulación de la respiración puede hacerse desde un punto de vista clínico, con la medida del tiempo inspiratorio, del flujo medio inspiratorio, de la PaCO2, y de la relación Vems/PaCO2, o con sencillos test de estimulación central, reinspiración, hipercapnia o hipoxia. Estos tests tienen una importante limitación en la alteración de la mecánica respiratoria del paciente; para evitarla, se han propuesto pruebas prácticamente independientes de ellas, como la presión de oclusión, o el electromiograma diafragmático.

La realización de estos test tiene interés fisiológico, fisiopatológico, diagnóstico y terapéutico, por lo que es conveniente realizarlos, ante la sospecha clínica de hipoventilación alveolar de origen central.

En este trabajo se presentan datos de la valoración, clínica y con sencillos test, de la regulación de la respiración y la exploración funcional pulmonar de un caso con hipoventilación alveolar primaria.

Summary

The exploration of the regulation of respiration can be made from a clinical point of view with the measurement of inspiratory time, average inspiratory flow, of PaCO2, and of the relation Vems/PaCO2, or with simple tests of central stimulation, reinspiration, hypercapnia or hypoxia. These tests have an important limitation in the alteration of the respira tory mechanics of the patient. To avoid them, practical tests independent of them have been proposed, such as occlusion pressure or diaphragmatic electromyography.

The results of these tests are of physiological, physiopathological, diagnostic and therapeutical intest, and therefore it is convenient to carry them out when there is a clinical suspicion of alveolar hypoventilation of central origin.

In this paper, the authors present data of the evaluation, clinical picture, and with simple tests, of the regulation of respiration and functional pulmonary exploration of a case with primary alveolar hypoventilation.
