La recidiva diafragmática de un timoma es un hecho excepcional. Se presenta el caso de una mujer de 70 años de edad, tratada 4 años antes por un timoma invasor mediante timectomía transesternal y radioterapia adyuvante, que ingresó por recidiva del timoma en el hemidiafragma izquierdo e infiltración del lóbulo inferior del pulmón ipsolateral.
Se llevó a cabo una exéresis en bloque que incluyó el hemidiafragma izquierdo, la pared torácica con tres arcos costales y una segmentectomía atípica del lóbulo inferior izquierdo, con reconstrucción del hemidiafragma y del defecto de la pared torácica con mallas sintéticas. Se completó el tratamiento quirúrgico con radioterapia adyuvante.
Diaphragmatic recurrence of a thymoma is rare. We report the case of a 70-year-old woman who underwent transternal thymectomy and adjuvant radiation therapy, and who was admitted four years later with recurrence of the thymoma in the left hemidiaphragm with infiltration of the inferior ipsilateral pulmonary lobe.
The thymoma was excised along with the left hemidiaphragm, chest wall and three ribs and an atypical segmentectomy of the left lower lobe was performed. The hemidiaphragm was reconstructed and the chest wall was repaired with synthetic mesh. Surgery was complemented with radiotherapy.
