Seguimiento a corto y largo plazo del neumotórax espontáneo tratado por videotoracoscopia
Short- and long-term follow-up of spontaneous pneumothorax treated by video-assisted thoracoscopy

Se analizan los resultados del tratamiento por videotoracoscopia del neumotórax espontáneo con 83 procedimientos en 79 pacientes (58 varones y 21 mujeres), con una edad media de 28,3 años (16-76 años). Las indicaciones fueron en 53 pacientes por neumotórax recidivante, en 10 por neumotórax contralateral (uno intervenido en ambos hemitórax), en tres por neumotórax bilateral y en 13 por fuga aérea persistente. En 7 pacientes (8,4%) hubo que reconvertir a toracotomía abierta. En la mayoría de los 76 procedimientos restantes, en 72 pacientes, el abordaje fue sólo por videotoracoscopia (88%). En 3 casos (3,6%) hubo que ayudarse de una toracotomía videoasistida o "de apoyo". La estancia media postoperatoria fue de 5,1 días (2-24). La mortalidad ha sido nula y hubmorbilidad operatoria en 3 casos (3,9%). Hubo morbilidad postoperatoria significativa en 9 casos (11,8%). Un paciente con fuga aérea prolongada fue reintervenido a los 12 días por toracotomía videoasistida. De los 72 pacientes hemos podido revisar 71, con un seguimiento medio de 28,1 meses (54 días-54 meses). Se han encontrado 3 recidivas (3,9%) y ningún caso de dolor crónico que requiera analgesia.

En conclusión, la videotoracoscopia es una abordaje eficaz con las ventajas de ser mínimamente invasivo, por lo que creemos debe ser de elección, tras mejorar las cifras de morbilidad y recidivas, que están influenciadas por la curva de aprendizaje.

The outcome of video-assisted thoracoscopic treatment of spontaneous pneumothorax was analyzed. Eighty-three procedures were performed in 79 patients (58 men, 21 women: mean age 28.3 years, range 16 to 76 years). The reasons for intervention were recurring pneumothorax in 53 patients, contralateral pneumothorax in 10 (one of whom was treated on both sides), bilateral involvement in 3, and persistent air leakage in 13. Seven patients (8.4%) also required open thoracotomy. In 72 (88%) of the remaining 76 procedures, only video thoracoscopy was used. Three patients (3.6%) underwent video-assisted thoracotomy. Mean postoperative hospital stay was 5.1 days (2 to 24 days). No related deaths occurred but surgical complications were reported for 3 (3.9%). Significant postoperative complications developed in 9 cases (11.8%). One patient with prolonged air leakage underwent a second procedure, video-assisted thoracotomy, 12 days after the first intervention. Seventy-one of the 72 patients received follow-up examinations, with a mean followup period of 28.1 months (range 54 days to 54 months). Three recurrences (3.9%) were recorded but there were no cases of chronic pain requiring analgesia.

We conclude that video thoracoscopy is an effective approach, with the advantage of being minimally invasive. We therefore believe it should be the procedure of choice, once improved morbidity and recurrence rates are observed, as these factors are influenced by the learning curve.
