Presentamos un caso de rotura de bronquio principal derecho por traumatismo torácico cerrado, tratado quirúrgicamente con éxito a los 10 meses del accidente, mediante resección en manguito de la zona cicatricial estenótica secuela de la rotura, y anastomosis término terminal del bronquio principal a nivel de su origen traqueal.

Comentamos la rareza de este tipo de lesión traumática con supervivencia, así como la técnica quirúrgica empleada, su indicación cuando el pulmón atelectásico no presente lesiones infecciosas irreversibles y los buenos resultados clínicos radiológicos y funcionales obtenidos.

We report one patient with right primary bronchus tear due to blunt trauma of the chest successfully surgically treated 10 months after the accident, by means of cuff resection of stenotic cicatricial zone, which was a sequel of the tear, and termino-terminal anastomosis of primary bronchus at its tracheal origin.

Rareness of this kind of traumatic lesion with survival as well as surgical technique employed, its indication when atelectatic lung does not present irreversible infective lesions, and good clinical, roentgenologic, and functional results are discussed.
