Se presenta un caso de mediastinitis fibrosa de larga evolución, con un amplio período intermedio de estabilidad clínica, que mostraba además una inhabitual invasión y destrucción miocárdica. Se discute sobre las similitudes y diferencias de comportamiento entre la granulomatosis mediastínica y la mediastinitis fibrosa, con especial hincapié en la argumentada hipersensibilidad del paciente a los antígenos del agente desencadenante del proceso.

We describe a patient with fibrous mediastinitis with a long clinical course who presented an intermediate period of clinical stability. The patient had also an uncommon invasion and destruction of the myocardium. We discuss the clinical similarities and differences between granulomatous mediastinitis and fibrous mediastinitis. We also emphasize the hypersensitivity of this patient against the agent that originated the process.
