La leptospirosis es una enfermedad infrecuente, que puede expresarse con diversas formas de presentación, entre las que sobresalen la afectación hepática o renal. Con respecto a las manifestaciones respiratorias, la mayor parte de comunicaciones refieren efectos de poca trascendencia, no siendo habitual que éstas dominen el cuadro clínico.

Se presenta el caso de un varón de 22 años, con leptospirosis icterohemorrágica, que presentó fiebre, mialgias, anemia, plaquetopenia e ictericia, pero en el que la hemoptisis e insuficiencia respiratoria destacaron al principio de la evolución. Se comenta la buena respuesta al tratamiento instaurado con doxiciclina.

Leptospirosis is an uncommon disease that may have several forms of presentation. Among them, hepatic and renal involvement are the most important. Respiratory manifestations rarely dominate the clinical picture and have not been considered of relevance in most of clinical comunications. We report the case of a 22 years old male with icterohemorrhagic leptospirosis who presented fever, myalgia, anemia, thrombocytopenia, and jaundice but also hemoptysis and respiratory failure from the beginning of the clinical course. We comment the good response to the treatment with doxycycline.
