Complicaciones infecciosas toracicas tratadas con mioplastias y omentoplastias

En los tres últimos años, 30 pacientes fueron tratados con mioplastias y omentoplastias para el manejo de diferentes tipos de infecciones torácicas.

En 11 pacientes se utilizaron para el cierre de distintas fístulas bronco-pleuro-cutáneas, en 16 pacientes por presentar mediastinitis postesternotomía, en 2 pacientes tras perforación esofágica torácica y en un caso de osteomielitis primitiva de pared torácica.

En total se han confeccionado 58 plastias: 31 mioplastias de pectoral mayor, 2 de pectoral menor, 9 de dorsal ancho, 6 de recto anterior del abdomen, 2 de serrato anterior y en 8 ocasiones de epiplon mayor.

En 23 pacientes la cicatrización se produjo en el postoperatorio inmediato. En 6 pacientes recidivó la infección, 4 de los cuales cerraron por segunda intención y 2 fueron reintervenidos satisfactoriamente. Un paciente falleció en situación de shock séptico.

La utilización de mioplastias y omentoplastias ha repercutido en el mejor tratamiento de las complicaciones infecciosas postquirúrgicas.

During the last 3 years, 30 patients were treated with myoplasty and omentoplasty for the management of thoracic infections.

They were used to close several types of broncho-pleurocutaneous fistula in 11 patients, because of poststernotomy mediastinitis in 16, after thoracic esophageal perforation in 2, and for primitive thoracic wall osteomyelitis in one.

Overall 58 plasties were performed: 31 with pectoralis major, 2 with pectoralis minor, 9 with latissimus dorsi, 6 with anterior rectus abdominis, 2 with serratus anterior and, in 8 cases, with greater omentum.

In 23 patients, scar developed in early postoperative period. In 6 patients the infection relapsed. In 4 of them the spontaneous closure of the wound was delayed and 2 were successfully reoperated. One patient died with septic shock.

The use of myoplasty and omentoplasty has resulted in a better treatment of the postoperative infective complications.
