Se revisan 13 toracoscopias realizadas con el objetivo de practicar fenestraciones pericárdicas en pacientes con pericarditis de etiología neoplásica maligna. Aunque la serie es reducida, por el momento ya se pueden obtener algunas consideraciones que creemos de interés.

Cuando el derrame pericárdico es de naturaleza neoplásica, la mayoría de las veces acompañado de derrame pleural, la toracoscopia ha sido diagnóstica al permitir la observación y toma de biopsias no sólo de la pleura, sino también del pericardio. El problema se plantea ante la indicación de pleurodesis, pues la pericarditis recidivaba al cerrarse la fenestración por el propio pulmón. Este inconveniente ha sido solucionado practicando pericardiodesis combinada con pleurodesis.

Thirteen thoracoscopies carried out with the aim of performing pericardial fenestrations in patients with pericarditis of malignant neoplasic etiology are reviewed.

Although the series is reduced some considerations of interest have been reported.

When the pericardial effusion is of a neoplastic nature, usually accompanied by pleural effusion, thoracoscopy has allowed the observation and biopsy of both the pleura and the pericardium. With the indication of pleurodesis a problem becomes evident since the pericarditis had recurred upon closure of the fenestration by the lung itself. This inconvenience was solved by the performance of pericardiodesis combined with pleurodesis.
