The detection by labeled human anti-gamma globulins of bacteria coated with antibodies in the secretions of the respiratory tract has been proposed as a method for identifying bacterial infections of the lung. The aim of the present work is to assess its diagnostic value in sputum samples. Fourteen sputum samples obtained from patients with bacterial pneumonia and 14 sputum samples from patients with other bronchopneumopathies were studied. Sensitivity and specificity of the method for diagnosing bacterial pneumonia were 86% and 43%, respectively, suggesting that such method do not distinguish between bacteria infecting airways from bacteria infecting lung parenchyma, but it allows to differentiate bacteria contaminating sputum samples from those causing infection. In our series, the previous treatment with antibiotics did not modify the results. The most appropriate samples for being studied are those containing more than 25 leukocytes per field (x 100). Finally, there were no differences in the results obtained in each sample employing anti-IgG, anti-IgA, or anti-IgM. The time required for positivation did not have any influence on the result, suggesting that the local immune response of the lung was of secondary type.
