This is a prospective study of 100 cases of spontaneous pneumothorax devoted to analyze its relationship with environmental and constitutional factors, physical exertion and tobacco. The results suggest that an age lower than 30 to 40 years commonly separates the group of idiopathic origin from those secondary to pulmonary diseases. The abrupt fall in atmospheric pressure may favour the development of spontaneous pneumothorax; 82% of cases were smokers. Idiopathic pneumothorax frequently occurs in tall patients, aged 15 to 25 years, smokers, and in those who did not practise sports or hard physical activity. It has been suggested that a low perfusion of the pulmonary vertex may contribute to the development of pneumothorax in these patients.
