Se presentan 31 casos de alveolitis alérgica extrínseca (pulmón del granjero). La clínica de los pacientes fue con mayor frecuencia crónica (67,7%), aunque es de resaltar que un 34,4% presentaron reacciones inmediatas. Se detectó un patrón radiológico intersticial en un 80,8%. El funcionalismo respiratorio mostró una alteración ventilatoria restrictiva en un 60% y una alteración ventilatoria obstructiva en un 53,3%. Se comenta la validez para el diagnóstico del test de provocación inhalatorio (positivo en el 58,3%), la demostración de anticuerpos precipitantes por doble inmunodifusión e inmunoelectroforesis, las pruebas cutáneas con el alérgeno y el test de inhibición de la migración leucocitaria. Se resalta la validez de estas dos últimas pruebas para separar pacientes, de granjeros sanos. Dada la frecuencia de esta enfermedad en zonas de índice de pluviosidad elevado cabe suponer que en nuestro país esta entidad está pobremente diagnosticada.

This study included 31 patients with extrinsic allergic alveolitis (farmers lung). The chronic form of the disease predominated (67.7%) although there also were instances of the acute form (34.4%). Interstitial anomalies were seen in 80.8% of all radiographic images. Pulmonary function studies showed restrictive ventilatory capacity in 60% and ventilatory obstruction in 53.3%.

The validity of inhalation challenge tests (positive in 58.3%), demonstraron of precipitating antibodies with double immunodiffusion or immunoelectrophoresis, allergen skin test and leukocyte migration inhibition are discussed. The validity of the two last tests for distinguishing affected farmworkers from healthy ones is underlined.

Because the disease is frequent in areas with heavy rainfall, it is suggested that in our country the entity is often not correctly diagnosed.
