Resumen

Los autores describen un caso de hipoventilación alveolar primaria o hipersomnia con respiración periódica en una paciente de 26 años, causada por micrognatia. Se practicaron estudios hemodinámicos, poligráficos, cinefluorografía y monitorización de la tensión de la art. pulmonar, determinaciones secuenciales durante el sueño de SaO2 y pCO2, y ulterior traqueostomía, con estudios poligráficos post-tratamiento. Los autores demostraron que el factor fundamental en la hipoventilación alveolar y en la repercusión cardíaca era la obstrucción durante el sueño de las vías respiratorias altas. Los autores aportan los resultados de los estudios poligráficos antes y después de la intervención, y piensan que es un caso más de los que abonan la opinión que en el síndrome de hipersomnia y respiración periódica el factor más importante es el obstructivo. Se revisan algunos aspectos de la literatura en relación con el caso.

Summary

The authors describe a caseofprimary alveolar hypoventilation or hypersomnia with periodic respiration in a 26-year-old patient caused by micrognathia. Hemodynamic and polygraphic studies, cinefluorography, monitoring of pulmonary artery pressure, requential determinations of SaO2 and pCO2 during sleep, and ulterior tracheostomy with posttreatment polygraphic studies were carried out. The authors demonstrate that the basic factor in alveolar hypoventilation and cardiac repercussion was the obstruction of the upper respiratory airways during sleep. The authors present the results of polygraphic studies before and after the surgical operation, and they believe that this is one more case in which the most important factor in the Hypersomnia and Periodic Respiration Syndrome is an obstructive one. Some aspects of the literature on this subject are reviewed.
