Derrame pleural metaneumónico complicado. ¿Tratamiento agresivo?

Analizamos retrospectivamente la evolución de 47 derrames metaneumónicos complicados no purulentos en función del tratamiento inicial y valoramos si el tratamiento conservador es suficiente en este tipo de derrames. Para ello dividimos los pacientes en dos grupos según hubiesen recibido tratamiento con tubo de drenaje o con antibióticos exclusivamente. No había diferencias significativas en edad, duración de los síntomas previos a la hospitalización, ni características del líquido pleural; sí las había (p < 0,001) en el tamaño del derrame al ingreso, siendo inferior al hilio pulmonar en los que no necesitaron drenaje. La infección del espacio pleural se resolvió en 26 de los 29 enfermos tratados con antibióticos. En 2 pacientes hubo que colocar tubo de drenaje por no ser suficiente el tratamiento conservador. Observamos la resolución de los derrames metaneumónicos complicados con tratamiento conservador; en los casos en los que el tamaño del derrame era inferior al hilio pulmonar el drenaje inmediato no fue necesario.

We analyzed 47 cases of complicated non purulent parapneumonic effusion retrospectively with reference to initial treatment in order to ascertain whether a conservative approach to therapy is sufficient. Patients were divided into two groups, those receiving a drainage tube plus antibiotics and those taking antibiotics alone. There were no significant differences in age, duration of symptoms before hospitalization or nature of the pleural effusion. There was, however, a significant difference (p < 0.001) in the degree of pleural effusion upon arrival, with those not receiving drainage presenting effusion less than the size of the pulmonary hilium. Infection in the pleural space was resolved in 26 of the 29 patients treated with antibiotics alone; in 2 patients this treatment was insufficient and drains were placed. We found that complicated parapneumonic effusion was resolved with conservative therapy in cases where degree of effusion was less than the size of the pulmonary hilium. Immediate drainage was unnecessary.
