Se presenta un caso de enfermedad de Belçget (EB) en un paciente de 27 años de edad con cuadro clínico de fiebre, disnea y expectoración hemoptoica en cuya radiografía de tórax se apreció ensanchamiento hiliar derecho. Se practicó toracotomía exploradora y se observó una masa pulsátil en la arteria lobar superior compatible con aneurisma y mediante biopsia pulmonar se confirmó en el estudio histológico vasculitis necrosante compatible con EB. En la arteriografía pulmonar efectuada se observó una dilatación aneurismática en la bifurcación entre la arteria lobar media y lobar inferior derecha con trombosis de arteria lobar superior derecha y lobar inferior izquierda. El paciente ha recibido tratamiento con corticoides y azatioprina encontrándose clínicamente asintomático.
Behcet's disease in a 27-year-old male is described. The patient presented fever, dyspnea and hemoptysis. A chest X-ray revealed right hilar enlargement. Exploratory thoracotomy revealed a pulsating mass in the superior lobar artery consistent with aneurysm. Pulmonary biopsy confirmed necrotizing vasculitis of the tissue consistent with Behcet's disease. Pulmonary arteriography showed and aneurysmatic enlargement of the bifurcation between the mid and righ inferior lobar arteries, with thrombosis of the upper right and lower left lobar arteries. Resolution of symptoms was achieved through treatment with corticoids and azathioprine.
