We present the case of an anterior mediastinal abscess secondary to a spontaneous esophagic rupture, which showed a delayed growing, slight clinical repercussion, and a favourable response to antibiotic and symptomatic treatment and to aspirative drainage. We report this case because the location of the abscess was unusual, the clinical manifestations of the previous mediastinitis were scanty, and the clinical course was benign.
