El estudio abarca 60 casos de neumotorax tratados en el Departamento de Medicina Intensiva de la C.S.S.S. «1° de Octubre». En él se resalta la gran incidencia de los producidos por traumatismos torácicos, su posible manifestación retardada, la frecuencia de asociación con hemotórax y el valor pronóstico. Tanto en este grupo como en los otros tres estudiados (espontáneos, patológicos y yatrogénicos) se recomienda el abandono de actitudes expectantes si no se sustituyen por una estrecha vigilancia.

Se revisan los resultados de las técnicas de drenaje pleural así como las complicaciones bacterianas con este sistema.

Se pone de manifiesto la gran importancia de la asistencia respiratoria mecánica como desencadenante de neumotorax, especialmente en enfermos con neumonías necrotizantes o caviladas, sugiriéndose la conveniencia del drenaje pleural profiláctico en estos casos.

Finalmente, se describen las alteraciones electrocardiográficas transitorias encontradas y la tasa de mortalidad en los diferentes grupos.

This study includes 60 cases of pneumothorax treated in the Deparment of Intensive Medicine of the C.S.S.S. «l.° de Octubre.» The study emphasiez the great incidence of those produced by thoracic injuries, its possible delayed manifestation, the frequeney of association with hemothorax and the prognostic valué. In this group as well as in the other three groups studied (spontaneous, pathological and iatrogenic) the abandonment of waiting attitudes is recommended if it is not substituted by cióse vigilance.

The results ofthe techniquesof pleural draining are reviewed as well as the bacterial complications of this system.

The authors emphasize the great importance of mechanical respiratory assistance as a causing factor of pneumothorax, especially in patients with necrotizing or caven pneumonías and suggest the convenience of prophylactic pleural draining in these cases.

Finally, they describe the transitory electrocardiographic alterations found and the rate of mortality in the different groups.
