Hemos utilizado la diatermia a través del fibrobroncoscopio para la resección de seis tumores endobronquiales de características polipoides, dos malignos y cuatro benignos. Los dos malignos fueron carcinomas epidermoides de vías centrales que cursaban con importante compromiso ventilatorio. Utilizando únicamente el asa de diatermia y en una sola sesión se logró una importante paliación tras la resección de amplios fragmentos de tejido. De los cuatro tumores benignos, en tres (carcinoide bien diferenciado, pólipo bronquial y fibrolipoma) se obtuvo un resultado curativo y en el cuarto (carcinoide bien diferenciado) se efectuó una extirpación parcial que permite plantearse una cirugía con resección más limitada.

Dado que los fibrobroncoscopios no están acondicionados para el empleo de este tipo de energía, es preciso extremar las precauciones y seleccionar mucho las indicaciones. En ausencia de broncoiáser, la diatermia es una alternativa accesible y barata para el tratamiento de este restringido grupo de pacientes.

We have used application of diathermy through fibrobronchoscope for the resection of six endobronchial tumors of polypoid characteristics. Two were malignant and four were bening. The two malignant tumors were epidermoid carcinomas of central airways and originated ventilatory compromise. Using only the diathermic handle in an unique session we achieved an important palliative effect after resection of large pieces of tissue. In three out of the four bening tumors (differentiated carcinoid, bronchial polyp, and fibrolipoma) we obtained a curative result. In the fourth case (weli differentiated carcinoid) we performed a partial extirpation that allows to envisage a surgical approach with a more limited resection.

Since fibrobronchoscopes are not prepared for the use of this type of energy, it is important to extreme preventive measures and select all indications. In absence of broncholaser, diathermia is an accesible and inexpensive alternative for the treatment of this restricted group of patients.
