Oxigenoterapia continua domiciliaria. Valoración de los criterios de prescripción y seguimiento mediante un control domiciliario

La oxigenoterapia continua domiciliaria (OCD) forma parte esencial del tratamiento de la insuficiencia respiratoria crónica en pacientes con EPOC avanzada. Existe un interés creciente por esta terapéutica durante los últimos años, pero diversos estudios sugieren serios errores de prescripción y control con una mala relación coste/beneficio.

Se ha realizado un estudio en el domicilio de todos los pacientes (308) que realizaban OCD en el área de salud de Salamanca en abril de 1991 mediante un cuestionario de recogida de datos y una pulsioximetría basal. Se estudiaron 232 varones y 76 mujeres con una edad media de 71 años. La SaO2 basal media observada fue del 90%; 209 pacientes (67,8%) presentaron una SaO2 superior a 90. Únicamente se detectaron 16 pacientes fumadores.

Se encontraron diferencias significativas (p < 0,001) en la realización de gasometrías de control y en el cumplimiento terapéutico entre el grupo de pacientes diagnosticado y controlado por servicios especializados y el de pacientes atendidos por médicos no especialistas.

Es necesario establecer programas de OCD con participación de los servicios especializados y de la medicina familiar y comunitaria destinados a controlar con mayor rigor la OCD.

Continuous domiciliary oxygentherapy (DCO) is an essential part of the treatment of chronic respiratory failure in patients with advanced chronic obstructivo pulmonary disease. Growing interest in this therapy has been seen in recent years but different studies suggest the existence of serious errors in prescription and control with a bad cost/benefit relation.

A domiciliary study of all the patients (308) undergoing CDO in the health care area of Salamanca in April 1991 was performed with a questionnaire of data collection and basal pulsioxymetry. Two hundred thirty-two males and 76 females with a mean age of 71 years were studied. The mean basal SaO2 was 90% with 209 patients (67.8%) presenting SaO2 higher than 90. Only 16 patients were smokers.

Significant differences (p < 0.001) were found in the gasometries of control and in the therapeutic performance among the group of patients diagnosed and controlled by specialized services and the patients attended by non specialist doctors. Continuous domiciliary oxygentherapy programs must be established with participation of specialized services and family and community health services destined to control the same more closely.
