La presión media en la boca en los 100 primeros ms de la inspiración con la vía aérea ocluida (P0.1) es una medición indirecta de la actividad de los centros respiratorios. No existen demasiados aparatos comerciales automáticos, suficientemente validados, que permitan extender su determinación. Nuestro objetivo fue estudiar la validez y la precisión de un nuevo manómetro automatizado para medir la P0.1 y comparar su funcionamiento con el equipo habitual utilizado en nuestro laboratorio. El aparato de nueva fabricación consta de un transductor de presión de tipo semiconductor conectado a un convertidor analógico-digital. El equipo habitual es de confección artesanal y consta de un transductor Honewell, una tarjeta analógica-digital (Data Translation DT2801) para PC IBM-compatible y una hoja de cálculo estándar. Ambos realizan un registro continuo y con una frecuencia de muestreo de 100 Hz. Para el estudio de linealidad, exactitud, sensibilidad y precisión se utilizaron 2 columnas de 5 y 25 cm de agua. Para la evaluación clínica, se estudiaron 33 individuos, que acudían al laboratorio para estudio de su función pulmonar. A cada uno se les realizó cinco determinaciones de la P0,1. Los resultados del nuevo aparato mostraron una sensibilidad de 0,1 cmH20, una precisión expresada con el coeficiente de variación del 0% y una linealidad con un coeficiente de correlación (r = 0,999) respecto a ambas columnas. La diferencia media entre ambos aparatos en la evaluación clínica fue de 0,11 (0,46) cmH2O (p < 0,0001). El análisis de la concordancia confirmó un sesgo en la lectura respecto al equipo habitual aunque sin significación clínica (0,08 a 0,14 cmH2O). Podemos concluir que la comparación del nuevo aparato con el habitual dio buenos resultados y su comercialización podría permitir un uso más frecuente de la presión de oclusión en la clínica.

Our aim was to determine the validity and precision of a new automatized manometer for measuring P0.1 and compare the device to the apparatus normally used in our laboratory. The newly manufactured device consisted of a semiconductor type pressure transduce connected to an analog-to-digital converter. The usual apparatus used was constructed in our laboratory from a Honewell transducer, an analog-to-digital card (Data Translation DT2801) for an IBM-compatible PC and a standard spread sheet program. Both devices recorded a continuous signal at a sample frequency of 100 Hz. To determine linearity, accuracy, sensitivity and precision, two colums of 5 and 25 cmH2O were used. For clinical assessment, 33 patients scheduled for lung function analysis were tested. Five measurements of P0.1 were performed on each. The sensitivity of the new device was 0.1 cmH20; precision expressed as a variation coefficient was 0%, and linearity expressed as a correlation coefficient was r = 0.999 between the two columms. The mean difference between the two devices was 0.11 (0.46) cmH2O (p < 0.0001). Analysis of concordance confirmed a reading bias for the new device in comparison with the usual one, although the difference was not clinically significant (0.08 to 0.14 cmH2O). We conclude that the new device gives good results and its availability on the market may allow more frequent clinical application of occlusion pressure measurement.
