Nuestra experiencia en el tratamiento de las lesiones obstructivas de la vía aérea principal mediante fotorresección con láser Yag-Nd

Desde enero de 1992 hasta diciembre de 1997, hemos realizado un total de 164 fotorresecciones con láser Yag-Nd (itrio-aluminio-granate-neodimio) en 116 pacientes con lesiones obstructivas traqueales o bronquiales. De estos pacientes, 78 tenían neoplasias malignas, 5 neoplasias de malignidad intermedia y 33 lesiones traqueales inflamatorias. En 18 ocasiones, la fotorresección se indicó con carácter de urgencia. Todos los procedimientos se realizaron bajo anestesia general y preferentemente en ventilación espontánea. Los resultados inmediatos variaron según la naturaleza y localización de la lesión. En las tumoraciones con proliferación endoluminal, la fotorresección representó un tratamiento paliativo con el que se obtuvo una buena repermeabilización de la vía aérea, desde el punto de vista endoscópico, en el 70% de los casos. La supervivencia media de los 44 pacientes con lesiones malignas que pudieron seguirse fue de 29 semanas, con una mediana de 15,19 (rango, 1-120). En los pacientes con estenosis traqueales inflamatorias, se obtuvo una rápida repermeabilización traqueal que evitó en todos los casos la realización de una traqueostomía de urgencia. Un paciente con una tumoración maligna traqueal murió durante el procedimiento debido a un cuadro asfíctico por hemorragia traqueal.

We performed 164 laser resections with a neodymiumyttrium-aluminum-garnet (Nd-Yag) laser in 116 patients between January 1992 and December 1997. Seventy-eight patients had malignant neoplasms, 5 had neoplasms of intermediate malignancy and 33 had inflammatory tracheal lesions. Eighteen resections were emergency procedures. All resections were performed with the patient under general anesthesia and preferably breathing spontaneousiy. Immediate results varied according to the nature and location of the lesion. Treatment was palliative for tumors showing intraluminal proliferation, providing successful reopening of the airway as shown endoscopically in 70% of patients. Mean survival of the 44 patients with malignant lesions who couid be followed was 29 weeks, with a median of 15.19 (range, 1-120). The tracheas of patients with inflammatory stenosis were reopened rapidly and emergency tracheostomy was avoided in all cases. One patient with malignant tracheal tumors died during the procedure due to asphyxia related to tracheal hemorrhage.
