Hemos valorado en 68 niños (edad media: 9 años) el efecto del tratamiento prolongado (media: 255±117 días) con budesonida inhalada en aerosol y cámara espaciadora a dosis variables (media: 1,42mg/1,73m2/día) sobre la función suprarrenal, mediante el test de ACTH (250 pg i.v.) ampliado a aldosterona, con determinaciones basales de ACTH, actividad de renina plasmática, sodio y potasio. Los valores basales medios fueron: cortisol 313±138nmol/1 y aldosterona 0,25±0,20nmol/1. En 4 casos el cortisol basal fue inferior a 130nmol/1. La respuesta obtenida tras administrar ACTH fue normal para la aldosterona y para el cortisol fue parcial en 2 casos. Concluimos que: a) la determinación de aldosterona no aporta datos significativos en el test de ACTH y que b) en el 4,4% de los casos se observa alguna disfunción suprarrenal tras inhalación prolongada de budesonida en niños.

We evaluated the effect of long-term treatment (255±days) with budesonide aerosol inhaled from a spacer device by 68 children (mean age 9 yr) receiving variable doses (mean: 1.42mg/1.73m2/day). The effect on suprarenal function was assessed by ACTH test including aldosterone activity, basal ACTH levels and blood renin, sodium and potassium levels. Basal values were cortisol 313±138 nmol/L and aldosterone 0.25±nmol/L. Basal cortisol was under 130nmol/1 in 4 cases. The response to ACTH injection was normal for aldosterone and partial for cortisol in 2 cases. We conclude that a) measurement of aldosterone does not add significant information to the ACTH test and b) suprarenal dysfunction after prolonged inhalation of budesonide by children is found in 4.4% of patients.
