Resumen

Se han estudiado 47 enfermos afectos de Bronquitis Crónica. Un estudio funcional en reposo, comprendiendo: espirometría, análisis de la ductancia para el CO, y hemodinámica pulmonar, ha precedido la determinación de la PMT (potencia máxima tolerada). Esta se ha efectuado mediante un ejercicio triangular con progresión de carga de 30 watios/3 minutos. Se halla una excelente relación entre el consumo de oxígeno y la ventilación a nivel de la PMT y el VEMS, VMV directa y VEMS%CV. El consumo de O2 a nivel de la PMT estuvo excelentemente relacionado a la PAP y a la diferencia PAP PCP. La frecuencia cardiaca a nivel de la PMT no estuvo relacionada a la situación hemodinámica pulmonar en reposo. El autor concluye: a) el predominio de los factores ventilatorios en la limitación de la capacidad energética del B.C. b) La señalada participación de las alteraciones hemodinámicas, de distribución ventilatoria y recambios gaseoso en dicha limitación, y c) La validez y excelente aplicabilidad clínica de la PMT como parámetro sintético, defmitorio de la capacidad energética máxima.

Summary

The authors have studied the cases of 47 patients suffering from chronic bronchitis. A functional study at rest, consisting of: spirometry, analysis of ductance for the CO, and pulmonary hemodynamics, preceded the détermination of the MPT (máximum power tolerated). This was carried out by means of a triangular exercise with progression from a load of 30 watts/3 minutes. The authors found an excellent relation between the consumption of oxygen and the ventilation at the level of the MPT and the VEMS, direct VMV and VMS%CV. The consumption of O2 at the level of MPT was related excellently to PAP and to the difference PAP-PCP Cardiac frequency at the level of the MPT was not related to the pulmonary hemodynamic situation at rest. The author concludes: a) the predominance of ventilatory factors in the limitation of the energetic capacity of the C.B.; b) the notable participation of hemodynamic alterations, of ventilatory distribution and, of gaseous exchange in this limitation: and C) the validity and excellent clinical applicability of the MPT as a synthetic parameter, which defines máximum energetic capacity.
