Tras inserción de drenaje pleural 10F, conectado a −20<span class="elsevierStyleHsp" style=""></span>cm H<span class="elsevierStyleInf">2</span>O, el paciente comienza con tos irritativa, dolor centrotorácico, hipotensión, taquicardia y desaturación. Se estabiliza al paciente en la UCI, solicitándose nuevo TAC que muestra opacidades en vidrio esmerilado en pulmón derecho, compatible con edema por reexpansión pulmonar (<a class="elsevierStyleCrossRef" href="#fig0005">fig. 1</a>). El paciente no mejora y precisa conexión a ventilación mecánica invasiva. Tras 48<span class="elsevierStyleHsp" style=""></span>h se consigue la extubación. En la cirugía se objetivaron lesiones bullosas apicales y múltiples <span class="elsevierStyleItalic">blebs,</span> realizándose bullectomía y pleurodesis mecánica por videotoracoscopia con retirada del drenaje pleural y alta a las 72<span class="elsevierStyleHsp" style=""></span>h de la cirugía, sin complicaciones.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0010" class="elsevierStylePara elsevierViewall">El edema de reexpansión pulmonar, descrito por primera vez en 1853 por Pinault, es una complicación rara, generalmente inferior al 1%, que surge tras una rápida reinsuflación del pulmón por la evacuación de un neumotórax o derrame pleural. Alcanza cifras de mortalidad del 20%<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">1</span></a>. El diagnóstico es clínico, apoyado en pruebas de imagen. Su tratamiento radica en medidas de soporte, requiriendo CPAP o incluso VMI con PEEP en los casos más severos, como el nuestro.
