El derrame pleural eosinofílico es término aplicado a un líquido pleural con porcentaje de eosinofilia igual o superior al 10%. Aunque sus causas son extraordinariamente variadas, su asociación con carcinoma no es frecuente. Presentamos un paciente de 36 años que debutó con un derrame de estas características sin ninguna otra sintomatología y que se resolvió coincidiendo con la administración de corticoides, siendo catalogado de idiopático. 4 meses después reingresó con un derrame pleural metastásico de un adenocarcinoma que sólo la necropsia localizó a nivel pulmonar, demostrando también una invasión generalizada del organismo.

Se revisan los conocimientos actuales sobre la patogenia de la eosinofilia asociada a cáncer. Se discute la circunstancia del debut de la clínica con derrame pleural eosinofflico y su posterior desaparición antes de la manifestación definitiva del cáncer.

Eosinophilic pleural effusion, is a term applied to pleural fluid containing 10% or more eosinophils. Al-though the causes vary enormously, its association with carcinoma is infrequent. We present a 36 year old pacient who had a pleural effusion of these cha-racteristics in absence of any other symptomatology which resolved itself coinciding with the administra-tion of corticoids, being categorized as idiopathic. Four (4) months later he was rehospitalized with a metastasic pleural effusion of an adenocarcinoma which was demonstrated on autopsy as being pulmonary in origen and which also revealed generalized invasion of the organism.

We revise the current knowledge on the pathogene-sis of eosinophilia associated with cancer. The appea-rance of the clinical picture with the eosinophylic pleural effusion in its later disappearance before defi-nite manifestation of cancer is discussed.
