Resumen

Presentamos un caso de un tumor embrionario con predominio de estructuras del seno endodérmico, que consideramos encuadrable dentro del carcinoma embrionario. No hemos encontrado referido ningún otro caso en la literatura nacional. Está localizado en mediastino anterior y lleva una supervivencia en la actualidad de 9 meses. La respuesta al tratamiento fue aceptable, aunque no lo suficientemente satisfactoria, y fue valorada según criterios clínicos, radiológicos, broncoscópicos y analíticos, entre los que cabe destacar su control con la monitorización de las alfaproteínas, las cuales cursaron con niveles mucho más altos que los descritos en la literatura, y sus niveles como respuesta al tratamiento han descendido de forma muy acentuada.

Creemos por ello, que las determinaciones de alfa-fetoproteínas y gonadotropinas por radioinmunoensayo, así como otros test de inmunidad celular, pueden servir de índice, lo suficientemente precoz, para poner en evidencia una mala respuesta a la terapéutica, o bien orientar unos criterios terapéuticos más eficaces.

Summary

The authors presented a case of embryonic tumor with predominance of structures of entodermal sinus that they feel can be placed within the framework of embryonic carcinoma. No reference to any other case was found in the national bibliography. The tumor is located in the anterior mediastinum and at the time of writing this paper had survived for 9 months. The response to treatment was acceptable, although not sufficiently satisfactory, and was evaluated according to clinical, radiological, bronchoscopic and analytical criteria. Especially worthy of note was its control using monitorization of the alpha-proteins, which ran with mucho higher levels than those described in medical literature. In response to treatment, its levels decreased in a very accented way.

Therefore, the authors feel that the detenninations of alpha-fetoproteins and gonadotropins by radio-immune test, as well as other test of cellular immunity can serve as an Index, sufficiently precocious, to make a bad response to therapy evident, or rather to orient more effective therapeutical criteria.
