Se presenta un caso de bronquiolitis obliterante con neumonía organizativa (BONO) idiopática. Sus características clínicas, radiológicas y funcionales coincidían con las descritas en la literatura. La anatomía patológica evidenciaba ocupación bronquiolar e intraalveolar por formaciones pseudopolipoideas de tejido conectivo, con presencia de fibroblastos, fibras de colágeno e infiltrado linfohistiocitario. Destacamos la buena respuesta al tratamiento, encontrándose la paciente asintomática y con recuperación radiológica y funcional completas en la actualidad, a los diez meses de seguimiento. Se comenta el caso haciendo especial hincapié en el alto grado de identidad para su sospecha y en el diagnóstico diferencial, debido a la buena recuperación y buen pronóstico en la mayoría de los pacientes.

An idiopathic case of obliterative bronchiolitis with organizative pneumonia is reported. Its clinical, radiological and functional features corresponded to those reported in the literature. The pathological study disclosed bronchial and intraalveolar occupation by pseudolipoid connective tissue formations, with fibroblasts, collagen fibers and lymphohistiocytic infíltrate. We emphasize the good response to therapy: at present, the patient is symptom free and with complete radiological and functional recovery after ten months of follow up. The comments on the case insist on the high relevance of an adequate suspicion and differential diagnosis, owing to the good recovery and good prognosis in most patients.
