Resumen

La ventaja principal del empleo de fármacos nebulizados es que éstos se depositan directamente en el tracto respiratorio, con lo que se alcanzan concentraciones mayores de éstos en el árbol bronquial y lecho pulmonar con menores efectos secundarios que si se emplease la vía sistémica.

La eficacia de la nebulización depende de muchos factores. Entre otros, de las características del fármaco a nebulizar (tamaño de la partícula, forma, densidad y tensión superficial de ésta), de la anatomía de las vías aéreas, de la técnica de inhalación del paciente y del sistema de nebulización utilizado. Los factores que determinan el tamaño de la partícula producida por un nebulizador incluyen tanto las características de la solución como la velocidad de flujo del sistema de nebulización. Cuanto mayor sea la velocidad de flujo menor será el tamaño de las partículas de aerosol. El parámetro más utilizado para medir el tamaño de las partículas aerosolizadas es la mediana del diámetro aerodinámico de la masa de las partículas de aerosol. Las partículas de un tamaño comprendido entre 1 y 5 μm son las que más probabilidades tienen de llegar a los lugares adecuados del árbol bronquial y de conseguir el efecto terapéutico buscado.

Los equipos o sistemas de nebulización se componen de una cámara de nebulización donde se introduce el líquido a nebulizar y se genera el aerosol, y de una fuente de energía necesaria para hacer funcionar el nebulizador. Hay 3 tipos de sistemas de nebulización: nebulizadores ultrasónicos, jet y de malla. Los de malla son los más eficaces de los 3. También son menos voluminosos, más silenciosos y más rápidos, lo que se traduce en un mejor cumplimiento por parte del paciente.

Abstract

The main advantage of nebulized drugs is that they are deposited directly into the respiratory tract and thus higher drug concentrations can be achieved in the bronchial tree and pulmonary bed with fewer adverse effects than when the systemic route is used. The effectiveness of nebulization depends on many factors, including the characteristics of the drug to be nebulized (particle size, form, density, and surface tension), the anatomy of the airways, the patient's inhalation technique and the nebulization system employed, among others. The factors determining the particle size produced by a nebulizer include both the characteristics of the solution and the flow velocity of the nebulization system. The higher the flow velocity, the smaller the aerosol particles. The parameter most widely used to measure aerosolized particle size is the mass median aerodynamic diameter. Particles of between 1 and 5 μm are those with the greatest probability of reaching the site of infection in the bronchial tree and of achieving the desired therapeutic effect. Nebulization systems or apparatus are composed of a nebulization chamber where the liquid to be nebulized is introduced and aerosol and an energy source required to make the nebulizer work are generated. There are three types of nebulization systems: ultrasonic, jet and mesh nebulizers. Of the three, the most effective are mesh nebulizers. These nebulizers are also the most compact, quietest and most rapid, which translates into better patient compliance.
