Seudotumor inflamatorio multifocal del pulmón con buena respuesta a corticoides

Una mujer de 75 años ingresó en nuestro hospital con fiebre persistente, tos productiva y síndrome constitucional de 2 meses de evolución. La radiografía de tórax mostraba un infiltrado bilateral en forma de placas subpleurales con múltiples nodulos satélites en ambos campos pulmonares. Las muestras de biopsias tomadas por toracoscopia eran compatibles con seudotumor inflamatorio del pulmón. Se desestimó la resección pulmonar por el carácter bilateral de la lesión y se inició tratamiento con cortícoides. La respuesta, tanto clínica como radiológica, fue excelente, permaneciendo la paciente asintomática tras 30 meses de seguimiento con dosis muy bajas de cortícoides.

A 75-year-old woman was admitted to our hospital with persistent fever, productive cough and systemic symptoms of two months’ duration. A chest film showed bilateral infiltration in the form of subpleural plaques with múltiple sate- Ilite nodules in both lung fields. The biopsy specimens taken by thoracoscopy were compatible with inflammatory pulmonary pseudotumor. Lung resection was ruled out because the lesions were bilateral and corticosteroid treatment was instated. The clinical and radiologic response was excellent and the patient was asymptomatic after 30 months of lowdose corticoid therapy.
