We report a patient with right upper lobar bronchi atresia associated with parenchymatous tuberculosis, whose x-ray imaging presented initially as an anterior mediastinum tumor. Bronchoscopy and bronchography led to the diagnosis of right upper lobar bronchi stenosis; a differential diagnosis was made between primary and secondary (TBC) bronchial stenosis. The anatomical and pathological findings actually clarified the diagnosis.
