Las resecciones mínimas en el tratamiento quirúrgico del cáncer de pulmón

Cuarenta casos de cáncer de pulmón en estadio I (T1 N0M0, T2N0M0) con una media de edad de 59,9, rango entre 42 y 77 años, fueron sometidos a resecciones mínimas de pulmón. No se registró mortalidad operatoria, siendo la tasa de morbilidad del 2,5%.

La supervivencia actuarial ha sido del 90% al primer año, del 55% al tercero y del 46% al quinto; el índice de recidiva local fue del 5%, correspondiente a dos casos a los 10 y 28 meses de la intervención respectivamente, los cuales fueron reintervenidos, completando en ambos una lobectomía.

Las resecciones menores de pulmón constituyen una técnica de elección en el tratamiento quirúrgico de los cánceres en estadio I, dada la buena supervivencia que ofrecen, el bajo riesgo que acarrean y la facilidad de ejecución que encierran, especialmente si se utilizan suturas mecánicas automáticas.

Forty cases of stage I lung cancer (T1 No Mo, T2 No Mo) with a mean age of 59.9 years (range 42 to 77 years) underwent minimal pulmonary resections. Operative morbility was 2.5% with no mortality. Actuarial survival was 90% at the first year, 55% at the third, and 46% at the fifth. Index of local relapse was 5%, occurring in two cases 10 and 28 after surgery, respectively. Both cases were reoperated and underwent a complete lobectomy.

Therefore, minimal resections of the lung represent an elective technique for the treatment of stage I cancers since they offer a good survival, a low risk, and are easy to perform, specially when automatic mechanical sutures are used.
