En este trabajo se intentan establecer las indicaciones de la broncoscopia, rígida y flexible, como acto complementario de urgencia en cirugía. Sobre un total de 3.000 exploraciones endoscópicas se hace un análisis retrospectivo de las 304 broncoscopias urgentes en cirugía, de las que 284 fueron flexibles y 20 rígidas divididas en tres grupos: /.Intubaciones preoperatorias (57), II. Operatorias (16), y III. Postoperatorias (231). Después de un análisis de cada grupo y teniendo en cuenta la escasez de referencias en la literatura sobre algunos aspectos del tema, se tratan de deducir consecuencias que tengan alguna utilidad de aplicación práctica, respaldada por la experiencia recogida. Así en el grupo I se comprueba la gran utilidad de la ayuda del fibrobroncoscopio en la intubación naso u orotraqueal para anestesia general en situaciones anatómicas diñciles. El grupo II permite analizar principalmente las diferentes técnicas para realizar una fibrobroncoscopia con anestesia general en las mejores condiciones ventilatorias. En el grupo III se evidencian las ventajas de aplicación del fibrobroncoscopio tanto en el aspecto diagnóstico como terapéutico para el seguimiento y control postoperatorio de tráquea y bronquios así como la resolución de atelectasias y aspiración de secreciones retenidas, principalmente relacionadas con la cirugía torácica que supusieron el 88 % de las broncoscopias aspirativas urgentes. En el análisis del tiempo que medió entre el acto quirúrgico y la indicación de la broncoscopia aspirativa no encontramos diferencias significativas según el tipo de cirugía (pulmonar, cardíaca u otras). La tendencia progresiva a disminuir el número de broncoscopias aspirativas en nuestro centro podría ser debida a varios factores de los que el más importante sería probablemente la fisioterapia respiratoria.
