Objetivo

Este estudio se realizó para medir el efecto de utilizar dos nebulizadores con débito diferente sobre la reactividad bronquial a metacolina en niños asmáticos.

Métodos

Veintisiete niños asmáticos atópicos, de 6 a 14 años de edad, inhalaron metacolina de acuerdo al método de Cockcroft hasta que sus FEV1 disminuyeran un 20% con respecto del valor control postsalino (PC20). Las pruebas de provocación bronquial se realizaron usando dos modelos diferentes de nebulizadores tipo jet: Ava Neb 1780 y Hudson UP Draft II 1730, cuyos débitos fueron de 205±13 mg/min y 356±31 mg/min, respectivamente (p<0,05).

Resultados

En 9/27 pacientes (33%) hubo diferencias significativas (una o más concentraciones dobles) en sus PC20 cuando se cambió un nebulizador por otro. Sin embargo, 7/9 niños presentaron significativamente una menor reactividad bronquial cuando las pruebas se realizaron con el jet nebulizador de menor débito.

Conclusiones

Este estudio sugiere que el uso de nebulizadores tipo jet con diferente débito puede influir notablemente sobre los resultados en las pruebas de provocación con metacolina en niños asmáticos. Esto último podría introducir sesgos significativos cuando se realizan mediciones comparativas de la reactividad bronquial, tanto en un mismo individuo como entre poblaciones.

This study aimed to assess the effects of two different nebulizer outputs on airway responsiveness to methacholine in asthmatic children. Twenty-seven atopic asthmatic children aged 6 to 14 years inhaled methacholine according to the Cockroft method until FEV1% had decreased 20% with respect to saline inhalation (PC20). The bronchial challenge tests were performed using two different types of jet nebulizer: Ava Neb 1780 and Hudson UP Draft II 1730, the outputs of which were 205±13 mg/min and 356±31 mg/min, respectively (p < 0.05). For 9 out of 27 patients (33%) the PC20 values were significantly different (one or more double concentrations) for the two nebulizers. However, 7 of those 9 children showed significantly lower bronchial responsiveness when the challenge tests were conducted with the low output jet nebulizer. This study suggests that using jet nebulizers with different outputs can markedly influence the results of methacholine challenge tests in asthmatic children. Such an influence could introduce significant bias in the assessment of bronchial responsiveness in comparative measurements in a single individual or in populations.
